Previous game, Nier RepliCant happened long before Aliens and Machines attacked. There is huge cap between two games.
What we see in this episode happened, in relative terms pretty recently but still more than decades, if not hundreds of years ago.
This War has been ongoing for, if i remember right, thousand years.
The in game explanation of the blindfolds is that they're combat goggles that show information ( like the HUD). But it's symbolic,the soldier units have their eyes covered and the operators their mouths. Only the commander doesn't have any part of her face covered.

This previous No. 2 isn't from any previous game like it might look like, but from a Stage Play with real actresses playing the roles of the characters from this episode, this was later re-adapted into a manga that is currently airing, this episode taked content from those two materials to get into this episode. As a little fun fact, the previous NieR game taked place roughly 8.000 years before Automata.
Resistance and YoRHa aren't fighting on the same side because Resistance was betrayed, and left behind by the Bunker, so they are on their own trying to survive, that's why they don't trust anything Bunker-related like those YoRHa type androids.
As you pointed out, all the cast from this episode are girls, as the Stage Play is called "YoRHa Girls", there's another one featuring only male characters as well; "YoRHa Boys", but I don't suggest getting into it yet, as it contains massive spoilers from this point in the story.
About the big girl, it is explained that she has two cores, being the reason of her size; in the manga she gives one of her cores in order to save Lily from the logic virus; here she sacrifices her life to save Lily, she's a nice character.
About 4A (the YoRHa Android that just covers one eye), she explains that it is because she wanted to stylize her hair like that and with the regular blindfold she couldn't, so she just cut it it out lol. Blindfolds are goggles exclusive from the YoRHa types, they are ordered to not remove them in any cirscunstance, so those who actually do it is a symbolism of rebellion, and invidiuality.
So about the Flashbacks during Lily's hacking… is difficult to get into it but I will try to give a summary. Lily is some kind of escort mode, she's useless for combat, her role is to give emotional support for the members, but the fighting was getting out of hand, and the Captains abused her to relieve their stress, Captain Rose when realized this, she killed her abuser and escaped from that camp with Lily. About why Rose was so focused on that bullet during their hacking on Lily, or how did she got infected are things that I don't actually remember, I can't give you an actual answer lol.
The history of the previous game takes place several thousand years before that of Nier Autoamata, what the anime does is show events or characters that are found in the game with documents that one finds or with material outside the game such as manga or official books, and even so the anime is changing some things compared to the game, but so far they are doing a great job with the adaptation, I wish the anime had more impact
Here is a (not so) little story of how the story of the games link together:
There is a game called "drakengard". I don't remember if is the 2 or the 3 game that have AN ending, where the mc and the bad guy are teleport to another word, they fight and basically both die (it's a kind of bad ending, because you die but save everyone).
From that ending, after like 2000 years comes what happen in nier (nier 1 or nier replicant). It's a world where the result of the final fight of drakengard free something in the air that is like a virus. For what I know, from one of the endings of that game come nier automata, but after 11000 years (or something like that).
The link it's there, but it's sooooo little that you can play any game without play the others (that's what I did, only played automata).

I feel the anime is very rush to stuff a 2+ hour musical or hundred-page manga adaptation about this Pearl Harbour mission story into a single 20 mins episode. It's still a good adaptation to focus on Lily's backstory, I was shocked when number 21 try to cure Lily, the anime even showed Lily's memory of how she was abused day to day by her formal captain before captain Rose killed that male Android captain and took her as one of the family. If I didn't read that manga before, I don't know what those shocking images mean and probably feel nothing when anime shows her insecure response towards strangers, and probably shed fewer tears when Rose parts with her because Rose is not only a comrade but more like a motherhood/sisterhood between the two. Also the metaphor of removing the blindfold before they meet their ends.. This story is really worth a mini-movie or prequel OVA if financially justified!
In the game the resistance leader is Anemone not Lily, Yoko Taro change the story line.
In the game Anemone survived because she stay with number 21 and did not descend into the elevator. They bomb the server inside the mountain dan the elevator cave-in. So all that goes down the elevator are presumed dead including lily.
Number 21 ask for a mercy kill from Anemone because the logic virus has evolved since lily and her vaccine is render useless.
Since they made a minor change who survived and who died, I hope they make a connection on what happen inside the mountain with the main story, and how number 2 survived. The game did not give any back story on number 2, so I really hope to see more on number 2
